TMC Foundation

The TMC-Stroke Resource Center Stroke Transition of Care Program is made possible by a grant from the Tucson Medical Center Foundation. As a result of the Foundation’s generosity, more than 400 patients a year are called after they are discharged from TMC.  The phone calls are made by a Community Stroke Nurse who gathers information on how the patient and their caregivers are managing their lives as they transition from the hospital to living with stroke. The SRCSA Community Stroke Nurse not only gathers information about things such as medication management, follow up appointments and physical and emotional health, but also provides information on community resources to stroke families.

 


Hacienda at the River

As a result of the Hacienda at the River and the Stroke Resource Center of Southern Arizona Partnership, the SRCSA has a physical space to offer ongoing activities such as individual consultations, presentations and music, art and movement therapies. These activities are offered to the stroke community at large as well as to residents at Hacienda at the River.
